Attachment to the SIL Service Agreement — completed with the participant for each agreement and at every review.

Section 1. Supports and pricing

Prices are the NDIS Pricing Arrangements price limits current at the date of service (2026-27 shown as a guide; limits update each 1 July and this Schedule is re-issued). Public holidays are billed at the public-holiday limit. The example row is a guide only — delete it when completing.

The Schedule totals the amount per week and the estimated amount per plan year (weekly multiplied by 52).

Section 2. Weekly roster of care — summary

The live roster (workers, exact times, changes over time) is maintained in The Care Web platform and forms part of this Schedule; every generated shift is recorded against this participant. The summary grid records, for each day Monday through Sunday: day hours, evening hours, and whether there is a sleepover.

Section 3. What is included and what is not

Included: the supports itemised above, delivered per the roster of care, with workers screened, trained and insured by us.

Not included (and never billed to your NDIS plan by us): rent or board, groceries, utilities, personal spending, activity entry fees.

Changes: you can request a change to this Schedule at any time; changes are agreed with you per our Supported Decision-Making Procedure and a re-issued Schedule replaces this one. Where your funding or the NDIS price limits change, we re-issue this Schedule.

Agreement to this Schedule

The Schedule is signed and dated by the participant (or their representative) and by a named representative of Disability and Mental Health Care Pty Ltd.
